Description

Two separate events of uneven sidewalk on Martin Luther King between 3rd and 4th Streets outside Children's Hospital Oakland exit door. Our two leased addresses: 645 4th Street and 638 3rd Street. Uneven sidewalk has caused one trip and fall injury nearest the 4th Street/MLK uneven surface and reported via Worker's Comp 3rd Party Liability. A second area of uneven surface is located on MLK closest to 3rd Street. The area was previously neon painted in a fading yellow. A new Glo Spray Florescent Orange now highlights the area as a danger to pedestrians. Has caused an additional four staff trip and falls in last six weeks. Two large magnolia trees may have "root cause" along MLK. I have additional photos and witness statements as needed. Will also notify City Attorney.
